This video is a brief description of the services provided by Child Find
FDLRS Crown Child Find, in coordination with Clay, Duval and Nassau school districts, locates children ages 3 to 5 who are potentially eligible for services under the Individuals with Disabilities Education Act (IDEA) and links them with needed services.
Child Find services help to promote general public awareness of programs and services available for young children who have or are at risk of developing disabilities. Service coordination for diagnostic screening, training, and support is also provided.

Completion of the steps listed below may expedite the screening process:
STEP 2: You will be contacted by the Child Find Office to have an appointment scheduled with a Child Find Specialist
STEP 3: Please fill in these forms and email, submit or fax these to the Child Find Specialist before your scheduled appointment. (These forms will need to be downloaded and saved after they have been filled out before they are sent back to Child Find)
E: Submit a copy of your child's birth certificate
STEP 4:: If you have any evaluation documentation please submit to your assigned Child Find Specialist.
STEP 5:: Forms if request by specialist to complete and submit
