Population Served:

Infants and toddlers from birth to 36 months of age with suspected developmental delays or established conditions that could result in delays; all eligible newborns in area Neonatal Intensive Care Units.

Fees:

No charge to families for evaluation services; Medicaid and/or private insurance may be billed.  Funding for recommended services is available through Part C when there is no other funding source.

Baker, Clay, Duval, and St. Johns counties.

Description:

Early Steps accepts referrals from parents, physicians, teachers, and other professionals.  Early Steps screens all newborns in Neonatal Intensive Care Units, provides service coordination, and makes referrals as needed.  Services for eligible infants and toddlers include, but are not limited to:

multi-disciplinary evaluation

Development of a Family Support Plan (FSP)

referral to community providers for recommended services
